Use. Authority-owned meters may be used only to meter water from hydrants maintained and owned by the Authority, which are primarily located in the public right-of-way, and shall not be used with private fire line hydrants on private property. Use of a Customer-owned meter is permitted only as approved by the Authority, and provided such meter is installed and operated in accordance with the Authority's Rules and Regulations.

Deposit. A deposit of $1,250.00 is required for any use of an Authority fire hydrant, whether the meter is owned by the Authority or by the Customer. The deposit may be applied by the Authority to unpaid water use charges, rental fees, penalties, and any repair and/or replacement cost for damaged or lost meters or equipment, with any remaining deposit balance to be refunded to the Customer upon return of any Authority-owned meter in good working condition, payment of all outstanding charges, and satisfaction of all obligations under this Agreement.

Fees. For Authority-owned Meters, the Customer shall pay a weekly rental fee of $25.00 (any period from one to seven days equals a one-week rental).

USAGE FEE. Any water usage shall be billed at the Authority’s established third-tier retail rate, per 1,000 gallons of usage, as may be amended from time to time.

REPORTING METER READINGS. Whether Authority-owned or Customer-owned, all meter readings shall be reported to the Authority’s Billing Office by email to HYDRANT@DDCWSA.COM by the 10th of each month. The Authority may permit meter readings to be called in to Customer Service at 770-949-7617 or faxed to 678-631-1515. Upon request by the Authority, meters, whether Authority-owned or Customer-owned, shall be brought to the Authority’s Billing Office during regular business hours for a reading or inspection.

PENALTIES. Failure to bring in a meter when requested by the Authority or failure to report meter readings to the Authority’s Billing Office by the 10th of each month shall result in the following late penalties: (1) First Offense - $25.00; (2) Second Offense - $50.00; (3) Third offense - $100.00; and (4) Fourth Offense - $1,000.00. Repeated failure to bring in a meter when requested or to report meter readings by the 10th of each month for two consecutive months shall be deemed an illegal use of water and may result in additional penalties of up to $1,000.00 per occurrence per day, in addition to any late penalties listed above or other legal action. Further, the Authority may disconnect service, remove a meter, or terminate the Agreement if the Customer fails to comply with this Agreement or with the Authority’s Rules and Regulation, and any such action shall not limit the Authority’s right to recover all sums due and any applicable penalties.

For purposes of this Agreement, “Customer” means the person or entity named above.
The undersigned acknowledges and agrees to comply with this agreement and all other applicable Rules and Regulations of the Douglasville-Douglas County Water and Sewer Authority, which are incorporated herein by reference, as amended from time to time. I agree that this request may be stored electronically and that any electronic copy of this request, including my signature, will be deemed an original, admissible and enforceable to the same extent as a paper original.
